New Patient Forms

Below are PDF forms to print and complete in preparation for your treatment at Scientific Therapy. Please read and fill out as much as possible prior to arrival. We appreciate the time and effort you devote to complete the forms.

If you cannot download or print the forms from our website, contact us to email them to you, come by the clinic to pick them up, or just come a little early on your first day to complete them.

YOUR FIRST VISIT: dress comfortably. We will ask you to roll your pant leg up to your knee, so loose fitting pants are recommended. Please do not put any creams or lotions on your feet or legs. We will be discussing the therapy, expectations, commitment, etc, during this time. We will be glad to answer all your questions, too.

In addition to the completed forms, please bring your Insurance and/or Medicare cards, drivers license/ID and calendar. Please call us if you need directions to the clinic.
